Tryouts Registration FORMS and Check List!
1. USAV Online Registration
- Go to the North Texas Regions website and register for your 2009-2010 USA Volleyball membership.
- Follow the online instructions.
- During registration, select UNDECIDED as a club name. After you have accepted an offer from a club, you can go back to the website and affiliate yourself to that club
- Once the region office has received fees, they will update the record to reflect payment. Parents will receive an automated email stating their child is a member. Parents should print that email and take it to tryouts or they may log in and print a membership card.
- Athletes MUST be registered and recorded as paid prior to attending any tryout

3. USAV Medical History and Release Form (Click Here).

4. Texas Titans VBC Release and Waiver Form (Click Here)
Print legibly and complete all information
5. Copy of Player's Birth Certificate.
Required for age verification at USA Volleyball events. (Age definitions)
6. Tryout Fee Check or Cash: $75.
